How to Submit your Documents

  1. Download the Vendor Agreement Form below.

  2. Fill out the Vendor Agreement Form (PDF).

  3. Upload the Vendor Agreement Form back onto this page.

  4. Upload all other documents you might need to do business at CPFM. This includes:

    • Certificate of Insurance with City Park Farmers Market listed as a “certificate holder.” Our office address is 2850 N Adams St. Denver, CO 80205. $1M in minimum general liability coverage.(mandatory)

    • Colorado and Denver Sales Tax Licenses (mandatory)

    • Food Handling License (mandatory if serving/sampling food)

    • Anything else that you may need to be a mobile vendor in Denver (ie Liquor Sales Room Application, communication of being under the Cottage Foods Act, etc.)
